Why does my Outlook Web Add-in display startup errors and require Retry → Start before loading?

Issue Observed

Every time the Outlook Web Add-in is opened, Outlook displays the following error:

"Something went wrong and we couldn't start this add-in. Please try again later or contact your system administrator."

Clicking Retry proceeds to the next step.

A second message is then displayed:

"This add-in may not load properly, but you can still try to start it."

Clicking Start successfully loads the add-in.

After these two prompts (Retry → Start):

  • The login page is displayed.
  • The add-in loads successfully.
  • The add-in functions as expected.

Expected Behavior

The add-in should open directly without displaying either of these startup errors or requiring users to click Retry and Start.

Question

Has anyone encountered this behavior with an Outlook Web Add-in? What could be causing these startup errors, and what should I investigate to identify the root cause?
